Job Description

  • Essential Responsibilities:
    • Provides direct care to clients in their homes as delegated by their MIC plan and/or physical/speech/occupational therapist (PT/ST/OT) and in compliance with plan of care, preventing hospitalizations and decreasing or preventing inappropriate ER visits.
    • Assists with or provides personal care
    • Assistance with ambulation/transferring of clients
    • Assistance in the kitchen: preparing meals, feeding clients, washing dishes, wiping exterior surfaces, taking out trash
    • Assisting clients with dressing
    • Assisting clients with grooming: bathing, shaving with an electric or safety razor, hair care, skin care, mouth care, nail maintenance (no nail trimming)
  • Assists in reminding clients to carry out their prescribed therapeutic regimen under provider’s RX.
  • Maintains a clean, safe and healthy environment for clients assigned to their care.
  • Companionship and socialization
  • Outdoor activities of taking walks and outings
  • Provide client transportation to run errands
  • Supervise clients to avoid falls or other accidents.
  • Observes and documents client status and care/service
  • Assists client with structure of tasks and keeping clients on point with their post-hospitalization care/exercises.
  • Recognizes and handles emergency situations as directed until help arrives.
  • Participates in performance improvement activities.
  • Incorporates the MIC Vision, Model and Values throughout their practice
  • Monitors, educates, and collaborates with appropriate resources and agencies together with clients/caregivers and case managers in home setting.

Caregiver Requirements:

• At least 18 years of age

• Read, write, speak, and understand English

• Valid driver’s license and current automobile insurance

• High school diploma or GED

• Able to lift/move 50 lbs without assistance

• Register with the state as an HCA (Health Care Aide) - We can guide you through every step.

• Pass state Live Scan background check and Tuberculosis test

• Eligible to work in the United States and provide employment eligibility verification
